Did you know in the recently announced short term changes by the NDIA, Support Coordination will be available to all NDIS participants and their families to ensure they have the support they need to manage the impact of the Coronavirus?
This means that even if you do NOT have Support Coordination in your current plan, this support can now be accessed from the CORE supports funding category.
Our friendly and professional team of Support Coordinators currently have the capacity to provide this support.
Contact us if you or anyone you know would like to know more about how our Support Coordinators can help during this challenging time.
The role of a Support Coordinator is to connect participants with an NDIS plan to supports and services in the community. Our role as Coordination of Supports is to compliment your life and help put your NDIS plan into action!
A Support Coordinator will assist you to:
Your first meeting with Ability Options will be with our Engagement Manager. We will walk through your NDIS plan and assess how we can best support you in achieving your NDIS goals. We will encourage you to share information about yourself i.e. name, NDIS number, disability and people most important in your life, so we understand what goals you would like to achieve.
During this meeting, a Service Agreement will be developed. This outlines how Ability Options and you will work together. If you decide Ability Options is your chosen provider for Coordination of Supports, you will need to sign the Service Agreement. The Engagement Manager will then match you with the best Support Coordinator to meet your needs, goals and aspirations.
Next, your dedicated Support Coordinator will contact you and start your NDIS Journey!
Your matched Support Coordinator will contact you and coordinate a meeting. Your circle of supports are encouraged to attend this meeting to help compile a comprehensive picture of your life.
During this meeting we will talk about how to meet your NDIS goals, understanding your funded support categories and explain the do's and don’ts of Support Coordination. Together we will develop a unique, tailored and whole-of-life program that reflects your specific needs, desires and aspirations. Therefore enabling us to achieve the goals in your NDIS plan.
After the planning meeting, we will complete a one page profile so we can personalise your service and match you with the right support staff and services for you.
Your Support Coordinator will meet with you to determine how your goals are being achieved and will assist you in navigating the NDIS through connecting and coordinating informal, mainstream and funded supports in a service delivery environment. We will walk alongside your NDIS journey and are committed to help ‘make it happen’.
Once your supports and services are established, together we will monitor your goals, needs and aspirations.
We will keep records about how goals are being achieved so there is good information available for you to use when you review your plan with the NDIA. This usually happens once a year.
As part of the process of working with you, we aim to develop and strengthen your skills, capacity and confidence in taking control of your NDIS plan. Once you are comfortable with coordinating your NDIS plan, you have the choice to independently manage your own plan.
We will help you develop skills and confidence to take control of your NDIS plan. We aim to:
This is an individual process and everyone learns these skills in their own time.
