The Ability Options Support Coordination Team connect people with supports and services in the community aligned to an individuals’ needs and goals. 

What is Co- Ordination of Supports(COS)

Support coordination is allocated in your NDIS plan. It assists you in navigating the NDIS, by connecting and coordinating mainstream, informal and community funded supports. 

Our committed, experienced and connected support coordinators work for you. They understand that your needs can change both short and long term, and are flexible in either planning for your future or providing assistance when the unexpected happens.

We work in partnership with community connected Aboriginal and Torres Strait Islander employees as well as employees from culturally and linguistically diverse backgrounds, who are understanding of your individual culture, cultural beliefs and values.

Our support co-ordination team understand that the process of implementing your NDIS plan can be overwhelming and can support you to:

• Manage your chosen supports effectively to get the best outcome and value for money from your plan
• Work with other people in your circle of support (family, friends, service providers, therapists, community and mainstream supports) in order to meet the goals of your unique NDIS plan
• Support you to access the NDIS portal so you can monitor how your funds are being spent
• Monitor the progress and outcomes of achieving your goals
• Assist you to prepare for your next NDIS review meeting
• Build your capacity to co-ordinate your own supports in the future, if you choose.
